All SIRIs are then investigated by the healthcare provider and then reviewed by the Patient Safety Quality Team together with clinical leads.
The 2015 NHS England Serious Incident Framework builds on and replaces the National Framework for Reporting and Learning from Serious Incidents Requiring Investigation issued by the National Patient Safety Agency (NPSA, March 2010) and NHS England’s Serious Incident Framework (March 2013). The revised framework became effective from 01 April 2015.
On 01 April 2016 the statutory patient safety functions that were previously delivered within NHS England have transferred to NHS Improvement. These functions include the responsibility of the National Reporting and Learning System (NRLS) and the patient safety alerting systems. Existing policies, including the Serious Incident Framework 2015 and Never Event Framework 2015 remains unchanged and continues to apply, however support, guidance and new publications are available at https://improvement.nhs.uk/resources/never-events-policy-and-framework/
The two key operational changes within the new National Framework are:
1. The removal of grading
Under the new framework serious incidents are not defined by grade - all incidents meeting the threshold of a serious incident must be investigated and reviewed according to principles set out in the Framework;
A single timeframe (60 working days) has been agreed for the completion of investigations previously known as Grade 1 or Grade 2 incidents. Independent Investigations will have 6 months from the date the investigation is commissioned.
