We would really like to hear from you if you are:
- A patient receiving palliative/end-of-life care
- A carer, relative or friend of someone who has recently passed away
- If you know someone who is receiving or who has recently received end-of-life or palliative care
We appreciate that it can be a hard subject to talk about, which is why any insight you can give is so valuable to us and important in helping make services excellent for others in the future.
You can remain anonymous or leave contact details should you wish to. The responses you give will not be attributed to you, whether or not you supply contact details, and all responses will be confidential and used only in general form to provide an overview of care.
The information you provide will help us remove variation in care, and commission services that meet the needs of patients better.
The purpose of the survey is to allow us to seek the view of people's experiences of end-of-life care, what it felt like for relatives and family members, and whether people felt that they had the capacity to ask questions or voice concerns, and if dignity compassion and respect was demonstrated.
Based on the responses given, the results will feed into the work of the End of Life Strategic Advisory Group, where actions for improvement will be taken forward. Positive elements will be emphasised and shared with relevant services, helping to inform the commissioning of end-of-life services in the future.
The survey is available online until Saturday 1 April 2017.
